Every athlete must have all of their e-forms for 2026-27 on athleticclearance.com and an updated doctor's clearance. Please reach out to Athletic Director Chris Neier at cneier@smmusd.org with any questions.

All students must have a doctor's physical and have completed the e-forms athleticclearance.com to be able to tryout and practice. See Forms for Athletics for more details.
